Abstract:INTRODUCTION: One of the weak points in the strategy for eliminating leprosy is the poor quality of screening. To overcome this, the World Health Organization (WHO) encourages endemic countries to run campaigns for the elimination of leprosy by circulating educational messages and mobilizing the medical community for early screening of cases. The aim of our study was to identify the motives for consultation with high predictive value for the diagnosis of leprosy and to determine the late diagnosis factors and hence assist the staff on site to improve the results of their leprosy elimination campaigns. PATIENTS AND METHODS: The study consisted, during the second trimester of 1999, in interviewing all the patients consulting for the first time the Marchoux Institute or the units screening for leprosy in the Bamako area. The interview recorded the motives for consultation, the delay before consulting and the reasons for late consulting. To assess their positive predictive value, the motives for consultation were related to the diagnosis retained (leprosy or not). RESULTS: One thousand one hundred and seventy seven patients were interviewed. The motive for consulting, "suspected leprosy", scored the highest positive predictive value (PPV) (80 p. 100): 12 cases of leprosy were diagnosed by 15 consultants having suspected leprosy. Neurological problems were the second motive for consultation (PPV=61.9 p. 100). The most frequent motive for consultation was spots or "macules" (20 p. 100 of consultations), but only provided a positive predictive value of 19 p. 100. Prior consultations and non-specialized treatments were identified as factors of delay in diagnosing leprosy (P<0.001). CONCLUSIONS: Diagnosis of leprosy cannot be based on the motives for dermatological consultation alone. The macules are the most apparent signs, but of low predictive value. Nevertheless, they are an early but non-specific sign of leprosy and are often neglected by the patient. Other than macules, attention must be paid to the neurological signs (dysesthesia, motor disorders) when screening for leprosy. These signs may appear early on, or be observed at a late stage in the progression of the disease. (AU).

Abstract:Agranulocytosis developed in 16 U. S. soldiers in Vietnam who were receiving daily dapsone prophylaxis for falciparum malaria. The were eight deaths in the group. All patients had been on prophylaxis from 3 weeks to 3 months. recovery or death due to sepsis generally occurred within 9 days. The toxic effects of dapsone have resulted in a limitation in the use of dapsone for prophylaxis of falciparum malaria in U. S. troops in Vietnam.(AU).

Abstract:Lymphocyte transformation has been used to study the immune response to Mycobacterium leprae aomng contacts and non-contacts of leprosy patients. Of 26 subjects living a leprosy endemic area for less than two months none responded to M. leprae; 24% of subjects who had lived in an endemic area for more than a year gave a positive response to M. leprae; more than 50% of individuals with occupational contact of leprosy for more than a year responded; and about 50% of caontacts of tuberculoid and treated lepromatous patients responded to M. leprae, while only 22% (4/18) of contacts of leproamtous patients treated for less than six months responded. It seems that leprosy is more highly infectious than is indicated by the prevalence of the disease and that a subclinical infection commonly follows exposure to M. leprae. The relatively low response found in contacts of active lepromatous patients suggests that in these contacts "superexposure" to M. leprae can bring about a decrease in host resistance.(AU).

Abstract:Sequential estimates of the levels of circulating immune complexes (CIC), complement catabolic fragment C3d, complement-mediated immune complex solubilization (CMS) and immunoglobulins were made in 24 newly diagnosed with borderline tuberculoid leprosy over a 20 month period after initiation of chemotherapy. Fourteen of these patients had not suffered from reversal reactions either at the time of presentation or during the follow-up. The levels of CIC were evaluated in them from the third to the eleventh month after starting chemotherapy and immunoglobulin G (IgG) levels were evaluated up to eight months. The concentrations of C3d and immunoglobulins A (IgA) and M (IgM) were normal in these patients. The other ten patients had reversal reaction at the time of diagnosis which subsided by the third month after starting treatment. They did not have reversal reactions later. The levels of CIC and IgG were elevated and those of CMS were depressed throughout the study period. Serum C3d level was initially elevated but came down to normal by the third month while IgA and IgM levels were within normal limits. The relevance of these findings to the genesis of reversal reaction is discussed in this communication.(AU).

Abstract:The chemotactic responses of neutrophils derived from 52 patients with lepromatous leprosy and 24 healthy controls were assessed in vitro by a modification of the Boyden chamber method using lipopolysaccharide-activated serum as a chemotactic factor. Highly bacilliferous patients with active disease of long-standing duration had significantly reduced chemotactic responses, while patients with burnt-out lepromatous leprosy as well as those with recently acquired disease showed normal chemotactic responses. Preincubation of normal human blood group O leukocytes in plasma from patients with active disease of long-standing duration markedly reduced their chemotactic responses whereas preincubation in plasma derived from patients belonging to the other two subgroups of patients did not affect much leokotaxis. These findings suggest that the diminished chemotactic capacities among certain plasma factors sucha as immunoglobulins or antigen - antibody complexes that may-by attachment to the cell membrane-block the receptors required for interaction with the chemotactic factor. Abstract:A relatively mild nonoliguric acute renal failure developed early in the course of daily rifampin treatment in a 54-year-old man who was receiving concomitant steroid therapy. The renal lesion was an acute interstitial nephritis characterized by tubular dysfunction:inability to concentrate urine, increase in fractional excretion of sodium and uric acid, and glycosuria with normal blood glucose values. All abnormalities resolved promptly upon withdrawal of rifampin while steroid therapy was gradually reduced. The documents the fact that rifampin-induced nephropathy can occur in the course of a continuous daily regimen and may be favorably influenced by steroid administration.(AU).

Abstract:This study covers 654 patients of leprosy who were predominatly of non-lepromatous type. Detailed examination of eyes has been done and ocular manifestations noted. Ocular manifestations with a bearing on status of treatment, extent of disease and duration of disease have been studied. The salient changes include sluggish corneal reflex, loss of eyebrows, madarosis and nodules on eyebrows. Patients suffering for 4 years and more manifested damage to iris, cornea and lens. the article stresses the importance of routine examinations of eyes (including slit lamp examination) at the beginning of therapy and periodical check-up thereafter. It also emphasizes that the non lepromatous patients also exhibit eye changes to a sizeable degree, through not to the same extent as lepromatous variety.(AU).

Abstract:Eight of twenty armadillos (Dasypus novemcinctus L.) developed severe lepromatous leprosy 3 to 3.5 years after inoculation with viable Mycobacterium leprae. A total of 988 grams of lepromas containing and estimated 15 to 20 grams of leprosy bacilli ahs been harvested from these animals. The large amounts of material now available will permit in-depth studies of the biochemistry and metabolism of the leprosy bacillus, and the animal model should make possible definitive studies on the immunology, chemotherapy, and epidemiology of the disease.(AU).

Abstract:In the Green-Pastures-Leprosy-Hospital in Pikhara/Nepal 340 outpatients with leprosy were examined to evaluate the incidence of lagophthalmus and posterior synechiae of the iris. Patients who had already received chemotherapeutical treatment during the first 5 years following the onset of the disease developed fewer cases of lagophthalmus in both types of leprosy than patients who remained untreated for longer than 5 years. A comparable positive influence of early chemotherapy on the incidence of posterior synechiae of the iris could not be proven. The possible causes were discussed...(AU).

Abstract:The effect of rifampin therapy in leprosy was studied in two clinical short-term trials in which skin punch biopsy specimens were taken at regular intervals for the inoculation of mice in order to monitor the decrease in proportion of viable Mycobacterium leprae in the patients lesions. In a trial of rifampin in a dosage of 600mg daily, the bacterial viability feel to undectable levels in the first specimen taken after the start of theraphy (at 3-4 days in 4 patients, 7-8 days in 9, and 4 days in 2). Dapsone-treated controls required 20 to more than 112 days for the same change. In a trial of a single dose of 1,500mg rifampin, the viability fell to undetectable levels in the first specimen taken after the start of therapy also (at 3-5 days in all 14 patients).(AU).

Abstract:Continuous dietary administration of rifampin to mice with an established Mycobacterium leprae footpad infection reduced the bacillary solid ratio, with an estimated survival half-life of 5-6 days. In rifampin-treated immunosuppressed animals the survival half-life of solid bacilli, in the absence of host immunity, was 12-13 days. Clofazimine and B1912 produced a significant effect on solid ratio only after a lag period of apparently 100 days. The rate of action was considerably slower than that of rifampin. Intermittent (once monthly) administration of both drugs produced effects similar to those of continuous administration.(AU).

Abstract:Motor and sensory nerve conduction studies were performed in the distal part of the ulnar, median and radial nerves of 12 tuberculoid and 12 lepromatous leprosy patients, compared with 15 normal subjects. Slowing of sensory conduction velocity (SCV) was shown in all nerves with no difference between tuberculoid and lepromatous patients. The radial SCV slowing is correlated (P less than 0.001) with the clinical findings. Impairment of motor distal latencies was observed only in tuberculpoid patients. It is concluded that the radial SCV is the most reliable conduction test and is proposed as an early diagnostic test for leprosy.(AU).
